Il rapporto\u00a0<\/span><span style=\"font-size: medium;\">OMS<\/span><span style=\"font-size: medium;\">\u00a029.4.19 sulla resistenza agli\u00a0<\/span><span style=\"font-size: medium;\">antibiotici<\/span><span style=\"font-size: medium;\">\u00a0presenta uno scenario catastrofico, per la salute e l\u2019economia globale.\u00a0Fino a\u00a010 milioni di morti l\u2019anno nei prossimi tre decenni, con costi sociali e culmini di diseguaglianze paragonabili a quelli della crisi finanziaria 2008. (1)<\/span><\/span><\/span><\/p>\n<h4><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>Antibiotico-resistenza, il problema e le sue cause<\/b><\/span><\/span><\/span><\/h4>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>L\u2019antibiotico-resistenza<\/b>\u00a0\u00e8 un fenomeno che si verifica quando i batteri \u2018imparano a resistere\u2019 agli antibiotici una volta in grado di sconfiggerli. E quando i microrganismi un tempo considerati quasi innocui iniziano a resistere a diverse classi di antibiotici (multi-antibiotico-resistenza), il problema si fa serio. Incremento della mortalit\u00e0, degenze ospedaliere prolungate, indisponibilit\u00e0 di cure efficaci.<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>L\u2019esorbitante<\/b><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>costo<\/b><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0delle dissennate speculazioni dei colossi del farmaco, come sempre, viene esternalizzato sulla comunit\u00e0 globale. La quale per mezzo secolo ha foraggiato le casse di\u00a0<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>Big Pharma,\u00a0<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">acquistando\u00a0antibiotici come fossero caramelle per la tosse. Senza che nessuno, fino a pochi anni fa, sollevasse alcun dubbio sui costi individuali e sociali del loro abuso. Salvo soltanto aggiungere una scatola di fermenti lattici &#8211; alle prescrizioni o suggerimenti da banco &#8211; per \u2018ristabilire la flora batterica intestinale\u2019, in una visione fiabesca dell\u2019interazione delle armi chimiche col microbioma.<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>La zootecnia<\/b><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0ha a sua volta contribuito e tuttora contribuisce a causare e aggravare l\u2019antibiotico-resistenza, poich\u00e9 i farmaci veterinari negli allevamenti di \u2018animali da reddito\u2019 sono stati impiegati in via sistematica anzich\u00e9 localizzata, quale \u2018strada pi\u00f9 semplice\u2019 per prevenire l\u2019insorgere di focolai d\u2019infezione. Se non anche per compensare le carenze igieniche e \u2018migliorare le prestazioni\u2019 produttive. Il legislatore europeo \u00e8 perci\u00f2 intervenuto con una drastica riforma (<\/span><\/span><\/span><span style=\"color: #0000ff;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">reg. UE 2019\/6<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">), al preciso scopo di limitare al minimo indispensabile l\u2019utilizzo di antimicrobici e antibiotici negli allevamenti.<\/span><\/span><\/span><\/p>\n<h4><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i><b>&#8216;Non c\u2019\u00e8 tempo da perdere&#8217;<\/b><\/i><\/span><span style=\"font-size: medium;\"><b>. Rapporto OMS 29.4.19<\/b><\/span><\/span><\/span><\/h4>\n<p><span style=\"color: #000000;\">\u2018<\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i><b>No Time to Wait. Securing<\/b><\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>the Future from Drug-Resistant Infections.\u2019\u00a0<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">Il rapporto pubblicato il 29.4.19 da OMS deriva dalle attivit\u00e0 del Gruppo di Coordinamento delle Agenzie ONU dedicato alla resistenza antimicrobica (IACG, \u2018<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>UN Interagency Coordination Group on Antimicrobial Resistance<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2019. (2)\u00a0La gestione di una crisi sanitaria e socio-economica di impatto globale postula invero un approccio condiviso, secondo la visione \u2018<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>One Health\u2019<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">. Laddove i c.d. \u2018<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>Sustainable Development Goals<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2019 (<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">SDGs<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">), in agenda ONU 2030, rappresentano al contempo obiettivi e strumenti di sopravvivenza e risoluzione dei problemi. In particolare per quanto attiene ai diritti di accesso a cibo\u00a0e acqua\u00a0sicuri, premesse di salute e igiene.<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>Allo stato\u00a0attuale<\/b><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">,\u00a0si attribuiscono almeno 700 mila decessi ogni anno a malattie resistenti ai farmaci, 230mila solo a forme di tubercolosi che non rispondono alle cure. Le malattie comuni divengono incurabili e i protocolli \u2018salvavita\u2019 perdono efficacia. Le filiere alimentari a loro volta aumentano la fragilit\u00e0, a causa dell\u2019impatto dei patogeni multiresistenti sulla \u2018<\/span><\/span><\/span><span style=\"color: #0000ff;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>food safety<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2019 (sicurezza degli alimenti) e cos\u00ec sulla \u2018<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>food security<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2019 (sicurezza degli approvvigionamenti alimentari, vale a dire disponibilit\u00e0 di cibo per le popolazioni). Lo IACG prevede che la resistenza antimicrobica potr\u00e0 condurre 24 milioni di persone in condizioni di povert\u00e0 estrema, di qui al 2030.<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #ff0000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2018<i><b>La resistenza antimicrobica<\/b><\/i>\u00a0<i>\u00e8 una delle pi\u00f9 gravi minacce che affrontiamo come comunit\u00e0 globale\u2019<\/i>\u00a0(Amina Mohammed, Vice Segretaria Generale dell&#8217;ONU e co-presidente IACG)<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>Lo scenario di previsione\u00a0<\/b>ha una scala diversa, si preannuncia un\u2019epidemia senza precedenti. Antibiotici, antivirali, antifungini stanno diventando inefficaci. IACG riferisce a 10 milioni di morti ogni anno, a livello planetario, entro il 2050. Con prevalenza nei Paesi LMIC (\u2018<i>Little Medium Income Countries<\/i>\u2019) ma senza escludere le economie pi\u00f9 mature. Il fenomeno \u00e8 infatti registrato a livelli allarmanti in tutti i Paesi indipendentemente dal livello di reddito nazionale. Siamo pur sempre umani e continueremo a esserlo, tutti, a prescindere da polizze assicurative e conti correnti. Senza mai perdere, in ogni caso, la speranza di raggiungere quella copertura sanitaria universale proclamata finora invano dalle stesse Nazioni Unite e dai politicanti che vi partecipano. Parole vuote.<\/span><\/span><\/span><\/p>\n<h4><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i><b>&#8216;One health<\/b>&#8216;<\/i><\/span><span style=\"font-size: medium;\"><b>, la sinergia indispensabile\u00a0<\/b><\/span><\/span><\/span><\/h4>\n<p><span style=\"color: #000000;\">\u2018<\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i><b>One health<\/b><\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2019<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>.<\/b><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0Salute umana, benessere animale, sicurezza alimentare e tutela dell\u2019ambiente nei suoi vari aspetti &#8211; ivi compresi la gestione di rifiuti e delle acque reflue &#8211; sono temi intimamente correlati e devono venire gestiti in sinergia. Il rapporto IACG sottolinea la necessit\u00e0 di un approccio coordinato e sistemico, multisettoriale. L\u2019Europa ha gi\u00e0 intrapreso questo percorso, a partire dal Libro bianco per la sicurezza alimentare (12.2.2000). Ed \u00e8 tuttavia indispensabile svilupparlo, anche in ambito internazionale, su diversi fronti.<\/span><\/span><\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-40259 size-full\" src=\"http:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399.jpg\" alt=\"\" width=\"1242\" height=\"805\" srcset=\"https:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399.jpg 1242w, https:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399-300x194.jpg 300w, https:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399-1024x664.jpg 1024w, https:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399-768x498.jpg 768w, https:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399-150x97.jpg 150w, https:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399-696x451.jpg 696w, https:\/\/www.foodtimes.eu\/wp-content\/uploads\/2019\/05\/IMG_1399-1068x692.jpg 1068w\" sizes=\"auto, (max-width: 1242px) 100vw, 1242px\" \/><\/p>\n<p><span style=\"color: #000000;\">\u2018<\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i><b>L&#8217;abuso e l&#8217;uso eccessivo<\/b><\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>di antimicrobici esistenti nell&#8217;uomo, negli animali e nelle piante stanno accelerando lo sviluppo e la diffusione della resistenza antimicrobica<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2019 , evidenzia il rapporto IACG. Si devono istituire sistemi normativi pi\u00f9 rigorosi, sensibilizzare al corretto uso degli antibiotici, finanziare ricerca e sviluppo di nuove tecnologie per combattere la resistenza ai loro principi attivi, abolire l&#8217;uso di antimicrobici d&#8217;importanza critica e altri farmaci veterinari come promotori della crescita in zootecnia.<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #0000ff;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u2018<i><b>Le raccomandazioni del rapporto\u00a0<\/b><\/i><i>riconoscono che gli antimicrobici svolgono un ruolo critico nel salvaguardare la produzione alimentare, la sicurezza igienico-sanitaria e il commercio, cos\u00ec come la salute umana e animale, e ne promuove un uso responsabile in tutti i settori<\/i>.\u00a0<i>I paesi devono incoraggiare sistemi alimentari sostenibili e pratiche agricole che riducano il rischio di resistenza antimicrobica lavorando insieme per promuovere valide alternative all&#8217;uso antimicrobico, come stabilito nelle raccomandazioni del rapporto<\/i>\u2019 (Jos\u00e9 Graziano da Silva, FAO, Direttore Generale)<\/span><\/span><\/span><\/p>\n<h4><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>Salmonella e\u00a0<\/b><i><b>campylobacter<\/b><\/i><b>, aumenta la resistenza<\/b><\/span><\/span><\/span><\/h4>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>I recenti rapporti di EFSA<\/b><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0(<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>European Food Safety Authority<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">) e del Centro europeo per la prevenzione e il controllo delle malattie (ECDC) mostrano come gli antimicrobici usati per trattare malattie trasmissibili tra animali ed esseri umani (es. campilobatteriosi e la salmonellosi) stiano perdendo efficacia e l\u2019antibiotico-resistenza non mostri segni di diminuzione.<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>In alcuni Paesi UE<\/b><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0la resistenza ai fluorochinoloni (come la ciprofloxacina) nei batteri\u00a0del genere\u00a0<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>Campylobacter<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">\u00a0\u00e8 cos\u00ec alta che tali antimicrobici non funzionano pi\u00f9 per il trattamento di casi gravi di campilobatteriosi. Anche la salmonella\u00a0nell\u2019uomo \u00e8 sempre pi\u00f9 resistente ai fluorochinoloni, cos\u00ec come \u00e8 elevata la sua resistenza a diverse classi di antibiotici (MDR,\u00a0<\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>Multiple Drug Resistance<\/i><\/span><\/span><\/span><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">).<\/span><\/span><\/span><\/p>\n<h4><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>Antibiotici, i consumi in Italia<\/b><\/span><\/span><\/span><\/h4>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>Il rapporto AIFA<\/b>\u00a0(Agenzia italiana del farmaco) di marzo 2019, relativo al consumo di antibiotici in Italia, indica il nostro Paese tra i primi in Europa per consumo di antibiotici &#8211; se pure con un \u2018trend\u2019 in lieve diminuzione. Nonch\u00e9, drammaticamente, per i tassi pi\u00f9 elevati di resistenza e multi-resistenza. (4)\u00a0<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><strong>Il consumo di antibiotici<\/strong> in Italia \u00e8 stato pari a 25,5 dosi\/giorno ogni mille abitanti, in calo rispetto al 2016 ma ben sopra la media europea (21,7 dosi\/giorno ogni mille abitanti). Meritano attenzione, in particolare, i livelli di consumo dei farmaci da parte dei bambini.\u00a0<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #ff0000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>\u2018<\/b><i><b>Nel corso del 2017 il 41,4% della popolazione pediatrica<\/b><\/i>\u00a0<i>(0-13 anni) appartenente alle sei regioni analizzate (Lombardia, Veneto, Toscana, Lazio, Campania e Puglia) ha ricevuto almeno una prescrizione di antibiotici sistemici, e sono state prescritte in media 2,6 confezioni di questi farmaci.<\/i><\/span><\/span><\/span><\/p>\n<p><span style=\"color: #ff0000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>Si osserva un picco di prevalenza d\u2019uso del 50% (<\/i>somministrazione a 1 su due, ndr<i>) nel primo anno di vita del bambino, senza differenze tra maschi e femmine. Questo valore si mantiene pressoch\u00e9 costante fino ai sei anni di et\u00e0. La prevalenza diminuisce poi progressivamente fino ai 13 anni di et\u00e0, dove si attesta al 30%\u2019<\/i>\u00a0(AIFA).<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><i>Sabrina Bergamini<\/i>\u00a0<i>e Dario Dongo<\/i><\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\"><b>Note<\/b><\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: medium;\">(1) OMS (2019).
